Restrooms in Bars, Resaturants, and hotels are not terribly numerous. Generally, bars and restaurants will have one or perhaps two. Hotels I guess it depends on how fancy they are. Diffucult to find? No, not really. In hotels try the lobby. Bars and restaurants more often than not toward the back-- in fondo as an Italian might say.
In truth, here's the bathroom situation. In any Italian city there is a bar every 3 blocks. Not a bar in the American sense but an Italian Bar. This is the place where you will go in the morning for cappuccini and cornetti ( croissants), where you will go mid-afternoon for an espresso kick or perhaps a snack, early evening for an apertivo, and perhaps after supper for some gelato which some bars make on the premises ( look for produzione propria signs). In short, it's an italian social institution par excellence.
Not coincidentally it is also where you'll go to the loo when you're going about town. Simply walk in confidently and head to the restroom. Some folks think it's nice to make a purchase at the bar if you use its' restroom. Probably they're correct. But generally it is not necessary. If you like a glass of mineral water will go for about 30 centissimi and an espresso about 80. Do not, however, expect pristine facilities. While you probably won't run across an infamous turkish toilet ( hole in floor, footprints for your feet) it is quite necessary to carry your own TP. Buy or bring the little individual packs of kleenex ( fazzoletti in Italian).
In sum, don't worry yourselves. You're going to have a grand time.

Most cities and towns also have public restrooms where you pay a small amount. These are usually clean and have an attendant. You will see signs pointing to these. If you have driven to a city and parked in a parking lot, there will usually be a restroom in the parking lot.

If you just walk to the bottom, you ar elikely to find many closed doors. Ask first because many bars leave the toilet door closed in orer to avoid them being used in illicit ways. The barman will give you the keys. Alice Twain -- A Typesetter's day 3.0: Blog.

Mostly agree with Hannibal. Around Florence there are plenty of hotels, bars, cafes and restaurants, but I would never use their facilities without buying something nominal (at least a drink). I think it would be rude to do otherwise.
In Florence, like in the US, it would be unusual, but not impossible, to find a toilet that was out of paper -- never hurts to have a little packet of kleenex, but needing it would be the exception, not the rule.
In the historical center of Florence, there is one well-located cafe/bar in particular that comes to mind called the HotPot. It is a few blocks up from the Uffizi toward the Duomo. I wish I could remember the street intersection, but I can't. I mention the HotPot because in this particular area of Florence (between the Duomo and the Ponte Vecchio) toilets are somewhat hard to find. The HotPot is sort of a cafeteria -- not the type of place I would recommend for food -- but it has enjoyed my "patronage" over the years given its strategic location.
Diagonally (NE) across the intersection from the HotPot is the most amazing little panini shop -- nothing more than a large closet inset into the building, where you walk up from the street. Big favorite for a sandwich. I know it when I see it, but I'm sorry I can't remember its name or give better directions. Does anyone know this place?

More unsolicited information: For your first full day in Florence, I recommend the Original Florence Walk: http://www.artviva.com/originalwalk.html Great for orientation and a wealth of information you otherwise likely will not learn on your own. The other guided tours by artviva also are excellent. For example, if you are planning to see the Uffizi, book a tour with them. Well worth the expense to enrich your experience. I recall your earlier post about driving from Florence into Tuscany. Get a detailed map of the area where you will be driving (from any travel store or the travel section of a large bookstore). The more detailed, the better.

The "Courtesy Point" map is on the Comune di Firenze website. I would imagine it's readily available at the tourist info offices, too.

There are public restrooms that you pay but are clean and have tp in: 1.Santa Croce Square where the Capella Pazzi is 2.Via Anguillara near the motorcycle mechanic ask where "il meccanico is" 3. In piazza Signoria inside the Palazzo della Signoria. 4. Near the central market in Via Della Stufa 5. In the train station 6. At piazzale Michelangelo on the terracotta terrace below the stairs that face the Ponte Vecchio and on the Viale Michelangelo between Piazzale and the church San Miniato al Monte Bars are good but the more you drink the more you have to use the powder room. Vivoli the icecream place near Santa Croce has a nice restroom. I suggest that you always have some tissues with you especially if you are female. You can buy a pack of tissues at any Tabacchi shop for a few cents.
