If you provide more details, we might be better able to offer advice. eg. What plans have you made so far? How long do you have in each location? What are your interests? How many in your group? Car or no car?
Also, it is probably a good idea to post on the Italy and Rest of Europe boards.

Hi everyone. Thanks for your replies. I'm finding the planning process a bit daunting.
There are only two of us. We plan to stay in Spain for about a week and in Tuscany for about a week. We won't have a car, so I'm trying to find make sure we stay somewhere accessible by public transportation.

I've never been to Barcelona, but I'm sure it's pretty easy to spend a week there without a car.
Tuscany is a different issue. It's a large area with public transit not really geared to tourists. You could always consider spending a week in Florence, with maybe one or two day trips via train to Siena &/or Lucca. It would also be possible to arrange a day trip to some of the smaller places like Montalcino, Pienza, Montepulciano. You would definitely want to do more than one of these smaller towns in a day - the best way to do that would be to rent a car.
However, I don't think I would try to combine both Barcelona & Tuscany in 2 weeks. I would pick either Spain or Tuscany. If you wanted to experience a larger centre for a week, spend it in Barcelona or Florence. Then, spend the 2nd week in a more rural area getting around by a rental car. I've never been to Spain so I can't be of much help there - sure others can offer suggestions. There are a lot of Trip Reports on Tuscany, including mine below.
You don't mention why you won't have a car. If it's fear of all those crazy European drivers, you probably have much worse wherever you live.
Finally, if you are planning on going in May, you'd better start firming up some plans soon.
