I have no experience calling/ faxing outside of the US. I would like to fax a reservation for the Vatican's guided tour. The number the website provides is 06 6988 5100. Is there some prefix or country code that I need to dial first? Won't starting with a "0" lead me to an operator?
Also, I believe I will have to direct them to send my reply fax to a different fax line from the one I send with. Do I just give them my area code plus 7 digit number? I'm sure they deal with millions of requests from the US and will know precisely what to do, but I want to do my best to eliminate potential for confusion.

From the U.S., you have to dial 011 39 before the 06 and the rest.
I'm not sure what the access code from Italy to the U.S. is. Someone more knowledgeable than I am may know. If you don't receive more precise information, I agree that putting U.S.A. before the 3-digit area code and the 7-digit telephone number should be sufficient for the Vatican Museums.
Pauline has given you some great links, with lots of good info. Short answer: from Italy, you would dial "00" for the Int'l access code, "1" for the country code, then the area code and number. On any phone number, you can use the "+" convention to substitute for the International dialling code, so you could show your number as (for example) +1-999-555-1212. Country code for the US is "1".
