I'm afraid I can't be of much help for the Last Supper, but yes, if you're planning on going to the Uffizi, reserving tickets ahead of time is definitely recommended. We've had a few discussions on this recently, which you can find if you search through topics from the past few days. But, just go to http://www.uffizi.com and you can find the information you need. Afraid not-----The Friends of the Uffizi is only for museums in Florence.

Regarding the Last Supper, when we were in Milan about 3 years ago we stupidly had not thought of reserving ahead of time for this visit. We debated getting onto one of those 3 hour whirlwind tours of the city which included the requisite 20 minute stop and visit at the Last Supper. But it was SOOOO expensive and we were three people, decided to do it "next time". So far, there has not been a next time but I imagine there will, someday.
You can get info and reservations at all the city hotels there about these tours.

I found out that if you go through the website to purchase the Friends of the Uffizi membership, they will in fact send you the member cards. It takes awhile, so plan ahead.
