I've been old all my life; it's stood me in good stead. Stay in airport. Buy newspapers, have a meal, look at pretty girls; no shortage in Milan -- 4 hours is not that bad.

Robert, Have you already taken into consideration the time you'll need to be back at the gate for security/check in for your next flight? Is your layover really four hours or is it three or two? If you choose not to stay in the airport, one option would be to taxi or bus to one of the nearby little towns for a meal or a walk. If you look at the Slow Travel restaurant or hotel reviews for Lombardy, you might find something that sounds appealing.

Have had the layover you mention myself on multiple occassions. Problem is taking the train down takes almost the same amount of time so really your only choice is to accept the fact that you will be sitting at MXP reading a book or snoozing. Up side, is on the way home it takes only a few minutes to get on the plane and go through security (Florence being tiny).
